We see many patients with sciatica (pain running down the leg). There is no “one size fits all” answer to the question of surgery. i.e. – not all patients with sciatica should get surgery.

Unless you have red flags / serious underlying conditions that would need immediate surgery (such as cauda equina syndrome, tumors, infections, or severe lumbar disc herniation), surgery for sciatica may help, and might be worth considering. According to Kevin Ju, MD, here are some criteria that may warrant surgery:

  • Severe foot or leg pain plus progressive nerve symptoms, such as numbness/weakness.
  • Conservative therapies have been tried and failed, such as chiropractic, physical therapy, oral medications, and spinal injections
  • Leg function is significantly limited, impairing your personal or work functions

Dr Ju summarises, “Unless treating a medical emergency, surgery is typically considered after 6 to 8 weeks of nonsurgical treatments are tried.”
